Print Ready & Editable Printable Forms

This page is set-up for your convenience, so if you ever run out of forms or fliers, the documents on this page are available for you to use as Follows:

  • Some are ready to print "As Is"

  • Others are made available to you through a "Canva" template for editing with your information before you print

  • Follow the instructions shown next to any form you select that needs to be edited before printing.

  2. Once Canva template is opened, CLICK:"USE TEMPLATE FOR NEW DESIGN"

  3. Edit 4 areas: 1-Your Name; 2-Your Title, 3-Your Phone#

    4- Your Email & ( *5-Optional Additional Text)

  4. Once completed, save the file as: Your name-OSBSF-1

  5. Download as a PDF File by Clicking "Share" on top far left corner of screen: Then "Download" Then "PDF Print, Then "Download"

  6. Your Flier will download/be saved, to whatever folder you have designated on your computer/device to store Downloaded files.

  7. Print as many as needed (Standard 8.5x11 size)

  1. Click the link above to access this form *Note: There are "Two" 1/2 page fliers to edit as shown in sample.
